How to have your gift matched, dollar for dollar:

Donate online now and then email, mail or fax your completed matching gift form.

Download and print the matching gift form and mail with your check to the address below.

Unsure if your gift qualifies for the match? No problem. When we receive your gift we automatically check our records to see if it qualifies. If it does, we will send you the form with instructions. If it doesn’t, you’ve still supported the Phil!
